We would like to nominate Deb from Case Management for the DAISY Award. There are so many situations that come into mind when describing how Deb made a meaningful difference in the care of patients.
A homeless patient was admitted one day. It is so easy to look past our homeless patients but Deb took the time to listen to her story and help her get the resources that she needs to get back on her feet. Deb is always working above and beyond to find placement and help make their transition easy.
Deb is the rock of the Oncology department. She has facilitated so many great resources and placements for our patients. We had a patient with recurrent cancer who had decided to go home on hospice. The family was very upset and unsupportive of the patient's decision. Deb, along with the Palliative Care team, discussed with the family the importance of being there as a family during this difficult time. The family changed their way of thinking and rallied around the patient so that she would be able to go home and be comfortable. Deb has made such a positive impact on so many patients and families. She is beyond appreciated on our unit!
