A disabled veteran was struggling to get back and forth to dialysis for over one year, and it was becoming harder for him. The transportation available to him did not work well with his dialysis treatments, so he was driving himself. The added stress was taking its toll on his health. After a hospital admission for stroke- like symptoms, he had a follow up visit with his primary care doctor. It was there that he met Degy Philip, Care Coordinator, a nurse who "took the time to listen" to his concern about this dialysis issue.
The patient's sister wrote that "quite frankly, we had given up hope of ever resolving this issue". They had tried several different agencies and he didn't qualify for the assistance he required. Degy Philips contacted other people and put everything in place to solve the problem. In summary she wrote "Where others had been just going through the motions and giving us lip service, he showed true concern and gave it real thought and follow through so that my brother's and our biggest concern was resolved."
Degy Philip's action lifted what felt like a heavy weight on the chest of this patient and family. His professional competence and compassion earned him the DAISY Award.
