Denise Stracner, BSN, RN
A patient from another state was admitted to our hospital with osteomyelitis. He expressed deep concern about being discharged so he could begin to drive a two-day trip home and arrive in time for a funeral. It was essential for him to receive his IV antibiotics continuously, without any interruption in his treatment, however it was a priority of the patient that he not miss this funeral.
Denise jumped into action to facilitate a plan, coordinating a prescription through Opticare in Reno, arranging for a midline placement, and ensuring that follow-up care with his podiatrist was set up upon his return home. Although this seemed relatively straightforward, the entire process required several hours and numerous phone calls to ensure a safe discharge plan for the patient. This situation exemplifies how Denise puts the patient at the center of care, and she deserves recognition.
