Elaine Snoddy works in the Home Based Primary Care Program for homebound and elderly veterans. She has worked with patients to deliver quality, caring healthcare to those who cannot come to the facility for regular appointments. The program helps ensure consistent care to these veterans to help them remain in their homes rather than being placed in hospitals or long term care facilities. Her caring attitude has been offered 24/7 to answer urgent call requests, organize collaborative meetings, and creatively problem solve issues to keep patients from otherwise potential hospital admissions. She helps ensure that the entire team remains focused on veteran centered care and respects the veterans dignity and wishes for their care.
