Elizabeth Smart-Chapman (Liz) exhibited exceptional charisma caring for patient, a 77yo male with terminal cancer with mets. Patient was getting discharged home to be followed by hospice from Gilcrest. Patients wife and son were very anxious and confused. Liz took extra time to help the family and case management to arrange for medical equipment and transportation which was delayed for hours. She worked with the family to address their needs and patient was very pleased with his care. When patient was leaving he said "I will miss you all because all the help Elizabeth gave to me".
