A patient with juvenile diabetes had aged out of her pediatric endocrinologist's services. She was admitted to observation status on a weekend with DM OOC. A consult was placed for Diabetes Education. On Monday, Elva found that the patient had already been discharged. On Tuesday, the patient called the floor because she couldn't get one of her prescribed medications from a pharmacy. The call was transferred to Elva.
While keeping the patient on the line Elva reviewed the discharge medications. Elva called a pharmacy only to learn that the one medication had been back ordered. Elva telephoned another pharmacy and learned the medication was available. She called the first pharmacy and had the prescription transferred to the second pharmacy.
During the call with the patient, Elva learned the patient had been assigned a Medicaid HMO but did not yet have her card. Without the card, the patient did not know her PCP or her ID number. Elva looked up the relevant information and relayed it to the patient. After helping the patient with her prescription and primary care physician information, Elva was able to provide the patient with diabetes education over the telephone.
