Erica Meyers has done an exceptional job not only with this patient but with numerous patients. She is dedicated to ensuring that the patients feel supported and that she collaborates with the teams to ensure that the work gets done. She also has received positive shout outs from the teams that she is supporting. For this specific patient, he is extremely complex and is homebound because of his weight. She spent time listening to the patient and proactively engaged with his providers to be his voice. Since she has been working with the patient, he has lost over 100 pounds. Although he still has a lot of work to do, he is now engaged in his care, and Erica continues to work with the MVP Team, IP, Post Acute, DME, Transportation, etc., to see that he receives the care needed for him to have improved quality of life. She enables the patients to feel heard. I truly believe that at the end of the day, the patients feel that they have been advocated for.
