Farzana Shah, RN
I would like to recognize Farzana Shah for their exceptional dedication to patient and family-centered care.
In both the Cardiac Intake Center and CPRU, she consistently goes above and beyond in planning for each child's procedural needs, thoughtfully anticipating challenges and coordinating details in advance to ensure a safe, smooth experience.
She is skilled at identifying and clearly communicating social challenges- such as transportation barriers, caregiver availability, language needs, custody issues, or home circumstances- so the care team can provide appropriate, compassionate support to the patient and family.
Farzana has on multiple occasions assisted adult patients in working through social challenges and ensuring they had an adequate discharge plan prior to arrival for their procedure.
What truly distinguishes Farzana is the way she thoroughly explains the day-of-procedure process step by step to patients and families. Families have verbalized and expressed their appreciation for how well informed and prepared they felt from their pre-procedure visit.
She coordinates with multiple disciplines, including child life, social work, and various medical teams. Using clear, age-appropriate language and a calm approach, she empowers families with knowledge and confidence leading up to their procedure day. She has participated in multiple global medical mission trips outside of her responsibilities at CHOP.
Farzana has also executed the coordinating and planning of APP retreats to allow team members the opportunity to recharge and refresh. Farzana's proactive planning, attention to detail, and genuine empathy make a meaningful difference in the patient and family's experience. She is a trusted advocate for her patients and a valued collaborator within the care team.
She truly exemplifies excellence in pediatric care and is highly deserving of this recognition.
