She is an exemplary team member, and her patients often don't want to graduate from our program because of her wonderful care.
Genevieve ("Genny") is a Nurse Practitioner for the Hospital ReAdmission Reduction Program (HARP). She is an exemplary team member, and her patients often don't want to graduate from our program because of her wonderful care. For example, one of the patients she was following was admitted to a nursing home. When the nursing home care team was discussing the patient's plan of care with her, she told them to "Call Genny" to run the plan by her - that is how much she trusted and relied on Genny's opinion.
Genny also goes above and beyond in coordinating care with patients. For example, she has made sure patients have the correct medications when discharged from the hospital and helped patients coordinate with the pharmacy to get the correct medications to their house. For one patient whose heart failure and blood pressure regimen was changed, she discovered they were still taking the old dose and was able to get the pharmacy to fill the new script urgently, which helped avoid a readmission to the hospital.
Finally, she is an amazing advocate for our patients. For one of our patients, she worked with our CWA to advocate for an Elder Services investigation, given the state of the home she found when she visited the patient at home. In sum, Genny is an amazing team member, patient advocate, and provider who has made many patients' lives better by being a part of their care.
