I wanted to take a second to acknowledge the hard work and dedication that I have witnessed in my nurse. She consistently goes well above and beyond her duties and I have personally seen the impact that she made on her patients and their families. She takes each patient and their families and gives them individualized risk factor education so that they can make sound decisions about their lifestyle. She is approachable to her team and is such a great asset during stroke alerts. Here are just some of the examples of her going above and beyond that I wanted to make sure were recognized:

• Arranged outpatient follow-up at a hospital for port placement.

• Arranged outpatient repeat CT and follow-up appointment with neuro thereafter.

• Personally filled patient's pill organizer after multiple phone calls with the patient and emergency contact to clarify accuracy after discharge. Continues to take phone calls from patients and answer questions post-discharge.

• She continues to follow up with him to ensure that he is following his plan of care. Because of this, she was able to determine that he was taking his medication incorrectly.

• Personally delivered BP cuffs to patients’ apartments.

• She consistently takes phone calls months after patients have left, to continue educating and supporting them.

• In addition to these things, she is the first nurse in the inpatient APRN position and has been instrumental in developing and defining this role. This nurse does all of this with a smile and continues to uplift the 5SW team, as well as her colleagues. She is a great role model, and a wonderful asset to the Neurology team.
