Throughout all of this, Gina also developed a relationship with the mother and learned about significant stressors in their life, and provided outstanding family-centered care.
I nominated Gina Urbine for the DAISY Award because she often goes above and beyond in her care for patients and families, but one example is particularly exceptional. Patients who are referred to the Multidisciplinary Asthma Clinic (MAC) generally are high-acuity patients, with difficult to control asthma, and often have many contributing factors including challenging home environments.

An out of state three-year-old patient was referred to the clinic, and after reviewing the chart a few days before the visit, it was determined that the patient had “slipped through the cracks” and the referral and coverage were not approved, he also had several appointments scheduled in various specialty clinics that day. After Gina participated in several care conferences, it was determined to cancel the appointment and try to schedule them in a few months after everything was organized.

Gina called the mom to update her on everything, after speaking with her, Gina realized the little boy needed to be seen sooner rather than later. She tracked down the out-of-state PCP and an outside caseworker to begin the appropriate referral process. In the meantime, she found CPT codes, submitted NPIs, communicated with GI, aero clinic, RAVE, family navigator, and social work to create a smooth visit for this family. After several days of communicating and coordinating, the visit was approved, and the family was scheduled to travel to Colorado. Throughout all of this, Gina also developed a relationship with the mother and learned about significant stressors in their life, and provided outstanding family-centered care.

On the day of the clinical visit, Gina once again went above and beyond. She coordinated with dermatology to come to see the patient while they were in MAC clinic and facilitated a telehealth visit with immunology. After the visit, Gina is now coordinating a team care conference to help the family navigate all their future needs and she will continue to go above and beyond for this family. Gina‘s family-centered care and coordination modeled the mission and values of Children’s Colorado, and her teamwork, communication, positive attitude, and above all perseverance contributed to this patient continuing to receive the expert care he deserves.
