Grace Defante, RN

I work in virtual health, with remote patient monitoring, supporting patients for 3–5 months after inpatient stays through daily weight and vital-sign monitoring and weekly calls. Our team reinforces education on disease management, symptoms, medications, diet/fluids, and resources so patients can eventually self-monitor and communicate concerns to their physician. In working with many cardiology offices across Wisconsin and Illinois, I have never seen a nurse more compassionate, responsive, and committed to helping patients feel seen, heard, and supported than Grace. Grace consistently goes above and beyond in her communication, education, and follow-through, and she truly embodies what it means to be a DAISY nurse.
