Care during a hospital stay is so important, but when it's time for a patient to be discharged that extra care and attention to detail is essential. After almost 100 admissions to hospitals and following discharges many times something slips through the cracks. Grace is always extra attentive to discharge orders, making sure prescriptions are not just with the paperwork but that they match the doctor's discharge orders. She gently reminds you of things you might forget to pack, instructs on care with detail given to anything new to you specifically, and works extra hard to coordinate with driver, patient and family. We greatly appreciate this special nurse.
