Graycie Handley
Graycie was taking care of a patient who was having trouble with their urinary system and had just had their foley catheter removed. The patient went down for a KUB x-ray to see if any abnormalities could help identify why this patient was struggling. The patient returned to the floor, and the x-ray results populated. Graycie noticed the results and took a look. She became concerned when the x-ray report read that the patient had a suprapubic catheter and the bladder was completely drained, as they were expecting some distention and a full bladder. Graycie then checked on her patient and verified if they had a suprapubic catheter or had it in the past. The patient denied this, and that is when Graycie brought this to her nurse, and they were able to call back to the radiology department. The radiologist thanked them for finding the error, and the patient's chart was corrected as the x-rays were documented incorrectly. Graycie found a great catch and error in her clinical day and followed all the correct steps to verify the concerns.
