At around 10:30 am one morning, a patient presented to the ER after being discharged from a neighboring hospital around 8 am. They were discharged with conflicting and confusing instructions. Because of this, the patient was brought into our ER by a family friend, for help. The friend did not stay. The patient was stable but confused as to why he was here. The ER staff and Hannah spent the entire day investigating and arranging how to best help this patient. The patient was cleared medically from an ER standpoint fairly early in the day but did not have a safe discharge plan. While in the ER the patient became increasingly confused, related to his dementia. He was also COVID positive which complicated placement issues as well. This nurse went above and beyond in reaching out to facilities for placement, coordinating with the family, updating the Charge Nurse, proactively looking into transportation options, and even going as far as working on a backup plan with the family, in case all her other efforts were exhausted. In the end, that patient was transferred safely to a facility around 9 pm, all because of the determination, and efforts of Hannah, who was looking out for the best interest of her patient. Hannah Malone, did all of this throughout her 12 hr shift, while still caring for her other patients, precepting a new employee, keeping her patient safe in his confused state, and maintaining PPE requirements for the COVID positive status.
