While the team provides excellent, high-quality clinical care every day, their true commitment shines brightest when the conditions are at their worst. This winter season, in particular, has been challenging due to severe weather, including heavy snow, ice, heavy rainfall, and dangerously low temperatures.
As many of us were able to work from the warmth and safety of our buildings, these nurses were out on the road, driving in hazardous conditions, navigating slippery driveways, and lugging necessary supplies in and out of patient homes. Their dedication ensures that our most vulnerable patients receive the essential, time-sensitive care they need without interruption.
Beyond battling the elements, this team demonstrates remarkable adaptability by providing expert clinical care in highly diverse and often non-ideal environments. They do not have the standardized, controlled settings of a hospital or clinic. Instead, they navigate homes that may be cluttered, poorly lit, lack proper sanitation, or present personal safety risks.
They routinely find creative solutions to ensure patient dignity and effective treatment, from improvising clean workspaces to managing complex medical equipment in a small living room, all while respecting the boundaries and emotional needs of the patient and their family. Their ability to deliver high-level care under these unpredictable conditions is a testament to their professionalism and commitment.
The Home Health team's commitment extends beyond clinical duties; it is a profound demonstration of perseverance, compassion, and a deeply held sense of responsibility. By consistently putting the needs of their patients first, even when it means facing the worst elements of nature, they embody the spirit of the DAISY Award. They are truly heroes on the front lines of care.
When you do a quick Google search on what a home health nurse is, you find out they are licensed professionals (RN or LPN) who provide skilled medical care to patients in their homes, typically following hospital discharge, for chronic condition management, or for disability support. They perform assessments, manage medications, provide wound care, operate medical equipment, and teach patients/caregivers, aiming to improve health and prevent hospital readmissions.
But our home health team is so much more. They are a team of 10 field nurses and 4 intake nurses. Together, they provided exceptional care for over 1080 patients last year and performed over 11,000 visits. This is an exceptional commitment to our community and drastically more than most small teams can perform.
Patients and families often call to request certain care team members and will even leave current home health services when our teams become available. Our dedicated RNs are known for just doing one last visit or stretching their caseloads to service return patients.
This group is an essential part of our community partnership and works together as a team to provide care beyond our hospital walls.
