Ms. Jacqueline Andres is a valuable member of the Home Telehealth (HT) team. She is one of the primary point-of-contact HT Care Coordinators who is a resource to the team and is involved in the daily operations with data mining (led the team for FY22 with almost a thousand consults data mined), cross coverage, and enrollment into the program. She is eager to assist her colleagues and also help in mentoring new nurses in the department. She exhibits dedication and leadership in the department with a performance improvement project on chronic disease management (ACDC program). ACDC is a telehealth intervention for high-risk Veterans who maintain poor diabetes control despite engagement with standard diabetes care services. She is recognized for her outstanding efforts and contributions to the ACDC program. The ACDC project has been extraordinarily successful – not only have they lowered hemoglobin A1c by an average of 1.6% across hundreds of high-risk Veterans nationwide, but the program was also named an Office of Rural Health Promising Practice in 2020 and was selected as a winner of the 2018 Diffusion of Excellence Shark Tank competition.
