Jan is an unsung hero to our patients at Delta Health. She continues to give of herself tirelessly with the entire multidisciplinary team to ensure safe discharge. Through countless phone calls and connections, Jan is looking out for not only the patients but facilitating the staff nurses as well as coordinating with clerical staff to make smooth transitions for the patients our community serves. Many cases are extremely difficult with no options left, yet somehow she facilitates a resolution with the help of the entire team! This is not a solo sport however her experience and ability to navigate the process and mentor others are superior!!
