Jane Martin 2023, RN

Jane was nominated by her manager in recognition of an exceptional act of compassion and patient-centred care. Jane, who works as a discharge navigator nurse facilitated the discharge home of a patient who was at the end of life; but Jane did not only ensure that the wishes of the patient to be at home to die were respected, she also coordinated the discharge of the patient's wife who was on another ward in the hospital and who was also at the end of her life so that husband and wife could be together at home and receive end of life care together. In order to ensure the wishes of the couple and the family were met, Jane went above and beyond to ensure this happened, showed incredible dedication and compassion, and used all her expertise to ensure support packages of care were set up for two people in a short time frame which logistically can be a very complex matter to achieve. Through this act of dedication, compassion, and excellence Jane was able to allow an elderly couple to have their final wishes.
