Janee Wolters, RN
Janee and I recently had a shared patient who was readmitted the same day she was discharged due to home health care having concerns patient and family were unable to manage home IV antibiotics.
During this readmission, the patient and family were still set on going home with home health care, which presented some challenges, including home care having several stipulations about readmitting the patient to their services.
Over the course of two days, Janee was able to provide extensive verbal and hands on education to help ensure this patient and family was able to discharge home as they preferred.
From a case management perspective, knowing Janee was assigned to this patient and willing to go above and beyond was a relief. She was able to help get this patient home and prevent a prolonged inpatient stay.
