An unassigned veteran with a behavior flag (placed by another VA) presented to the Sidney clinic just prior to closing. This veteran was recently discharged from an MH facility and expressed interest in RRTP. He expressed an imminent housing crisis as well as transportation and financial concerns since recently traveling to the rural Sidney area. Janette and her team stayed late to ensure the veteran's concerns were addressed and to ensure team safety. Janette reached out to RRTP contacts both at the previous facility and at Cheyenne VA. She collaborated with the PACT RN and VSO to ensure housing for the night and to assist with facilitating contact with RRTP. She also placed a consult for PACT SW. Thanks to the efforts of Janette and her team, this veteran is being admitted to RRTP at his home facility. Further, her calm, professional demeanor and knowledge of VA resources ensured a safe, positive interaction with this veteran.
