Because of Jennifer’s work, our team can provide the seamless, extraordinary care that our patients deserve.
The Lowry clinic identified serval opportunities for improvement surrounding care coordination, follow-up, and provider communication. Jennifer took it upon herself to lead and develop comprehensive care coordination plans for patients at our Lowry specialty clinic. Because of Jennifer’s work, our team can provide the seamless, extraordinary care that our patients deserve.

Jennifer has worked with the Bariatric Program staff to determine the necessary documentation needed for patients to be approved for bariatric surgery. After insurance denials, Jennifer helped patients navigate the system to meet the requirements necessary to have their life-changing surgery approved.

For our Palliative Care Program, Jennifer coordinated patient families to meet with medical providers to discuss Medical Aide options. Working with a network of local hospice centers, she established timely transitions and enrollment for those in need. She then identified learning opportunities to advance her goals of care conversations with patients and families.

Jennifer also collaborated with Diabetes educators to obtain continuous glucose monitors for patients. Because of this, the endocrinology team can analyze patient glucose data to improve diabetes management and follow-up care.

If this wasn’t enough, As the co-chair of our Intensive Outpatient Clinic, Jennifer worked with providers, social workers, and clinic nurses to develop a program where high-utilizing patients could be seen the same day in an effort to decrease ER/hospitalizations. As a result, we have now seen a 33% decrease in ER utilizations, and a decrease of 11% re-admission rates between April 2020 and Jan 2021.

Lastly, Jennifer was selected as a research nurse fellow to develop a randomized controlled quasi-experimental trial to study care management facilitated visits as a way to decrease hospital and ER utilization. The research study is still in the recruitment phase but is a true testament to her contributions.  Jennifer, you have not only improved the lives of our patients, but also the lives of your team members.
