Primary Care Veteran X went to the ER after having an amputation of his right great toe. The Veteran also complained that his glucometer was working incorrectly to obtain his blood sugar. The ER obtained a glucose level, addressed his surgical site, and provided educational material regarding wound care, Hypoglycemia/Hyperglycemia, and a wound care consult was entered. The ER encouraged the Veteran to contact the PACT Nurse to follow up and obtain a glucose meter. The following day, Veteran X contacted the PACT Nurse, explained his need for a replacement glucose meter, and was offered a same-day nurse appointment. An appointment was made, and a Diabetes Education consult entered.

Nurse Chapman, who is the Glucometer Champion at the Virginia Beach Community Based Outpatient Clinic (CBOC) was respectful of the Veteran’s needs and immediately stepped into action to advocate for the Veteran. Nurse Chapman took the opportunity to educate the Veteran on how to operate the glucose meter, the Interventions for hypo/hyperglycemia, and the benefits of maintaining therapeutic blood glucose levels to improve the wound healing process. Nurse Chapman consulted with the Primary Care Provider and the PACT RN to get the Veteran's diabetes supply. Nurse Chapman encouraged the Veteran to obtain his blood sugar level, while at the clinic. She also explained to the Veteran how important it is to maintain a log at home. PACT RN followed up with the Veteran and he expressed sincere appreciation for the care and commitment that was provided by Nurse Chapman.

Because of Nurse Chapman's dedication to our nation’s Veterans and her dedication to the Veterans’ health and well-being, the Veteran avoided a medical process that would have otherwise been detrimental to his health. Nurse Chapman demonstrated compassion and did not hesitate to go above and beyond to assist the Veteran. Nurse Chapman is the ultimate team player, as she constantly assists her colleagues whenever the need arises. She is the epitome of the slogan, “World Class Service”. Nurse Chapman’s unwavering support and commitment extend beyond her scope of practice. Nurse Chapman has the highest level of passion, integrity, and excellence that is the main ingredient for a great Nurse, she is a valuable asset to the team and will continue to excel in her future endeavors.
