Jessica Segura exemplifies clinically and personally what it means to deliver consistent, exceptional care to patients and families. Jessica is also bilingual in Spanish.
Another nurse on our unit received a young female admission with multiple medical problems including non-compliance, insurance issues and an overall learning/resource barrier. The nurse asked Jessica to intervene with the language difficulty. Jessica had quite a conversation with our patient and got to the bottom of all of the patients issues and concerns. The patient made a connection with Jessica and Jessica continued to follow up with our young patient creating a personal plan of care and involving the care manager early in planning discharge. She ensured that the patient would have resources and medication necessary on discharge to continue to manage her medical conditions. Jessica performed extensive patient teaching about her medical conditions and the medications to manage these problems with both the patient and her mother.
On the day of discharge, Jessica went to say good-bye and the patient expressed concern about her anticipated problem with transport to the clinic for follow up. Jessica contacted the senior service transport services in the patient's town and, after explaining specifics, was able to engage their cooperation and assure that the patient could be driven to her clinic appointments. Jessica was convinced that adequate follow-up was key to the patient's success in managing her many health problems.
This is not the first time Jessica has gone above and beyond her responsibilities to ensure safe patient care. She is truly dedicated and engages her patients in education, demonstrates patience and is committed to providing quality nursing care. She is a boundless resource and role model on the unit.
