I would like to take this opportunity to tell you how much the Care Management Program has meant to our family. My husband D is your Stage 4 COPD patient. He has a long history of lung infections, ambulance rides, hospitalization, and nursing home stays. Each time he was discharged from the nursing home he was under the care of OSF home nursing for several weeks. He would make steady progress and eventually be deemed no longer in need of home nursing care. Everything would be fine for a time, but eventually, he would seem like he was starting to get sick again.
We were in the helpless situation of him being unable to travel to your office and no longer having a nurse coming to the house to check on him. Each time this happened he ended up in the hospital. After his latest hospitalization/nursing home stay/OSF home nursing cycle, we were once again left in the same situation. Then, we received a call from Jessica Snowden, the Care Manager. She informed us that she was going to be calling every week to check up on D to see how he was doing. She also said that we could call her at any time, and she would take any concerns we had directly to you. We were able to get some help with questions we had and also some advice for future situations.
One of those situations was getting a flu shot for D. I called Jessica and explained that I was worried about getting a shot for D. All the members of my family were vaccinated, however, due to his inability to travel I was unable to get D to your office or any of the flu clinics available to the public. She said she would look into it and get back to me. Within a few days, your office manager came to our house during the lunch hour and administered the shot to D. I just can't adequately express how grateful I am for that level of caring and concern. It means so much. Knowing that we can call for help when we need it is so comforting. It is very difficult and scary taking care of someone who has a serious illness. Knowing that there are professional, caring people that we can reach out to when we need help is a great blessing.
I would like to thank you and your staff for everything you've done to help us. The Care Management program is truly making a difference for us and I'm sure many other patients in this community feel the same way.
