Jin Choi, BSN, RN
Nomination #1: I would like to recognize Jin Choi for delivering truly exceptional, patient-centered care and advocacy. During a recent discharge, Jin identified a significant barrier to safe discharge: her patient spoke a native African language and was unable to read or write, and no one in the patient’s family could assist with written instructions. Rather than allowing this to become a safety risk, she took extraordinary initiative to ensure the patient fully understood how to care for himself at home.
She thoughtfully created pictograms to replace written discharge instructions, making the information accessible in a way that met the patient s needs. She also coordinated with a translator and completed return demonstration for a required injection, ensuring the patient could safely perform the task independently. When it became clear that additional reinforcement was needed, she consulted the unit-based pharmacist. Together, they continued problem-solving developing a color-coding system that matched discharge medications to pill bottles to simplify medication management.
Jin spent hours reviewing instructions, leveraging available resources, and thinking creatively to overcome communication and literacy barriers. Her persistence and dedication ensured a safe discharge and directly prevented potential complications or readmission. Jin exemplifies the highest standards of nursing practice through compassion, innovation, advocacy, and patient safety. I am proud to recognize her for going far above and beyond in service of her patient.

Jin expressed concern for patient safety involved with medications.
Jin spent over 2 hours with a pharmacist and an interpreter drawing out the medications; suns and moons and other symbols; and color coding his medications. She also brought in the case manager to set the pt up with resources to obtain a primary care physician. She would not leave this patient to discharge until she was POSITIVE he understood all discharge instructions.
