Jodi was caring for a patient that was comfort measures only and family anticipated their loved one would pass away soon that day. The family was at bedside, but after most of the day went on without a change in the patient's condition the family decided they wanted to take their loved one home. This was at 5:30pm on a Sunday evening when Jodi started to make referrals and get the pieces in place to fulfill the family and patient's wishes. She made referrals and collaborated with the palliative care team, hospice, worked with pharmacy, physicians and set up transportation via ambulance. The patient was able to go home by 7pm and spend her remaining time comfortable with her family.

Jodi was nominated by one of her colleagues for her patient advocacy juggling a complicated discharge and providing care to the rest of her assignment. Jodi went above and beyond to provide "something more"!
