My family decided that we needed to bring our father home. Johanna pivoted quickly from in-patient care to the discharge process. She had tremendous grace in managing his medications, communicating with physicians, specialists, and the others who would help bring our father home. Later same day, our father was home and smiled for the first time in days. We were able to have precious memories with him before he peacefully passed.
