It quickly became evident that Mr. G was having significant trouble articulating his thoughts through verbal communication.
John Bozovich walked up to the front desk to get something off of the copier. Our MSA’s telephone was ringing as she was currently assisting another patient with their concerns at the other window. RN Bozovich picked up the phone and Mr. G (a patient) was on the phone and was requesting information from his last appointment that was communicated to him but seemed to be having trouble articulating what exactly he was looking for. RN Bozovich advised Mr. G that he was at the front desk and that he would go back to his desk and call him to try and better understand what it was that he was requesting. RN Bozovich hung up the telephone completed current immediate tasks and went back to his desk and started creating a TCON for documentation purposes. His telephone rang and it was Mr. G. It quickly became evident that Mr. G was having significant trouble articulating his thoughts through verbal communication. This was confirmed by RN Bozovich by asking Mr. G “are you having trouble putting the words together to tell me what you want?" he replied Yes. “Do you know what you want to say but cannot get the words to come out?” he said Yes. RN Bozovich asked him is anyone else home? The patient said yes. RN Bozovich asked if it would be ok if he talked to the patient’s wife about his concern regarding the trouble the patient was having and asked if the patient could put her on the phone. It was quiet. RN Bozovich did not hear him call for her and proceeded to ask if it would be ok If RN Bozovich called her. He said Yes. RN Bozovich called the patient’s wife on her telephone and expressed his concern by asking Mrs. G if Mr. G was ok and stated that he was concerned about him having trouble communicating. She stated that she would check. A few moments later RN Bozovich could hear her say “John is on the Telephone for INT Med.” Then Mr. G was on the phone again. RN Bozovich asked Mr. G if he could talk to his son about my concerns. He said yes. RN Bozovich told Mr. G that he would call his son and that he needs to go to the emergency room to be evaluated. Then he heard Mr. G yell “J!”. His son got on the phone and RN Bozovich advised him that it seems as though Mr. G is having trouble communicating and that these symptoms can be signs of a stroke and that his father needs to go to the emergency room as soon as possible and that he should call 911. J confirmed that he would get him to the hospital. A care management follow-up call was conducted with Mrs. G confirming that Mr. G was kept in the hospital.
