Jordan repeatedly reassured this patient that by the time she was discharged, she would make sure that she felt comfortable with everything.
We recently had a younger patient on our floor that had a terminal cancer diagnosis and had made the very difficult decision to no longer pursue treatment, but instead focus on staying comfortable for the time that she had left. This patient could be difficult at times, but you would never be able to tell that by the way that Jordan handled herself. Jordan was very diligent and worked tirelessly with the patient, the physician, care management, and even the home hospice agency, to make sure that all of the patient's concerns and needs were going to be met prior to the patient being discharged. 

Jordan went above and beyond and spent many hours talking with the patient and her family, educating and explaining to them the medications that she would be going home on and how to administer them. This patient was very anxious since she would be going home with a PCA pump and IV pain medications. Jordan repeatedly reassured this patient that by the time she was discharged, she would make sure that she felt comfortable with everything. The day came for the patient to be discharged, her PCA pump had been delivered, and we were just waiting for the hospice agency to arrive to set her up on the pump. As time kept passing, and the hospice RN hadn't arrived yet, the patient and her family continued to get more anxious. 

At that time, Jordan took it upon herself to find the number for the hospice agency and figure out who was on call so that she could give this patient a more definitive discharge plan. Jordan had even stated that she would stay after her shift if needed to make sure that this patient got out of here without any issues. 

Once the hospice RN arrived, Jordan went into the room with her, helped her set up the PCA pump, and again reassured the patient that everything would be fine and that she would not let her leave until she felt comfortable. As the patient was leaving that evening, she couldn't thank Jordan enough for everything that she had done for her. It is thoughtful and caring nurses like Jordan, who make such a difference in the lives of our patients. 

I can say that I am truly honored to be able to work beside Jordan and am so appreciative of all of her hard work and dedication!
