Jordan was my grandmother's nurse case manager when she was admitted for a UTI and new metastatic disease. She decided that she was done with aggressive treatment and wanted to transition to comfort care. I am a nurse at WMC, and while I see this often, I have never dealt with it on the backside of things. Jordan would call me every day, multiple times a day, and keep me posted on her care, her options, and the things I needed to complete to get things moving on the hospice side of things.

All of the things that I wanted to do to get my grandmother into a long-term care facility Jordan had already done for me. She was attentive and empathetic and brainstormed a plan when the insurance companies were being difficult. She was continuously updating me and made me feel so comfortable when it came to long term goals of care. She was proactive and a constant blessing during my grandmother's stay. Warren Memorial and their patients/family are very lucky to have her in this role. This job can sometimes be thankless, and I want her to know how appreciative we are of all her hard work and that her compassion doesn't go unnoticed. Thank you, Jordan
