Jordon Moracco is an exceptional nurse who exemplifies going above and beyond for his patients. As a Care Coordinator, Jordon's role is to meet with discharging Veterans to ensure they receive the proper education, supplies, and in-home resources to make their discharge safe and successful.

When he works on a case, he goes above and beyond to ensure thorough and seamless communication.

Jordon recently worked on a case in which he dedicated many hours to coordinating with a Veteran, his son, providers, PM&R staff, PACT, and his Assisted Living Facility to ensure the Veteran was safe returning home. The Veteran's discharge was a success because of Jordon.

He is truly an indispensable part of the Care Coordination team and SAVAHCS!
