Judith Olapo, RN
I am honored to recognize Judith Olapo, RN, for her exemplary clinical judgment, attention to detail, and unwavering dedication to Veteran-centered care. Her actions during the care of Veteran Mr. E. directly reflect the VA’s ICARE Values.
During her overnight care of the Veteran, Judith demonstrated exceptional clinical vigilance. While performing routine but meticulous assessments, she identified an abnormal urethral discharge – a finding that none of the treating providers had yet detected. Her recognition of this subtle but concerning change prompted her to review the Veteran’s medical history in detail, where she noted his significant and relevant past history of Fournier’s gangrene.
This connection was critical, as the Veteran had presented with what initially appeared to be a simple urinary tract infection and had recently been discharged from the emergency department without this emerging complication being identified.
Through her thorough assessment and ensuring no finding was overlooked, Judith promptly alerted me to her concerns. Her keen observations triggered an immediate diagnostic workup, ultimately revealing a significant prostatic abscess. This finding required evaluation by Infectious Disease, Urology, and Interventional Radiology and led to timely, targeted treatment.
Had Judith not intervened, the Veteran likely would have been discharged home once again, placing him at high risk for severe infection, sepsis, or potentially fatal outcomes. Her proactive advocacy protected his health and ensured he received the urgent and specialized care he required.
Judith consistently practices with respect, not only in the way she cares for Veterans but also in the collaborative, dependable manner in which she supports the care team. Having worked with her for years, I deeply value her reliability, communication, and clinical insight. She never hesitates to remind me of important details, which directly elevates the quality of care we provide together.
Her longstanding pattern of thorough assessment, critical thinking, and compassionate presence demonstrates excellence in nursing practice. I trust her completely, and I know that when she cares for one of my patients, nothing will be missed.
