Over the past couple of years, the Care Management department was tasked with creating overage plans for patients over the age of 18 and transition plans for patients approaching the age of 18. Once those plans are complete, the patient is given resources and they are closed out of the Shriners system. This has proven to be very difficult for families both financially and emotionally. Judith helped to come up with the idea of giving the patients a certificate of completion. When the patients receive their certificate it gives them a sense of accomplishment and they are very proud.
