Julia came in to work (on a day she was AA'd) to help a patient and his family receive the 3 Wishes Initiative. This program, newly rolled out and not yet performed by the patient's primary RN and RN new employee Preceptee (Taylor Robbins), was to help the patient receive the utmost comfort and attention while in the dying process and on comfort measures. The decline of this patient happened rather quickly and was changed from an ICU status to comfort measures within only a couple of hours after admission to the CCU. The new employee nurse (Taylor Robbins) and primary nurse contacted Julia Wilde (who had organized and initiated the 3 Wishes Initiative Program and started it on CCU before it could be further introduced to other units) via telephone for more detailed instructions to implement this opportunity for the patient. Julia decided to come in to clarify the instructions, as well as explain to the patient and the family the 3 Wishes Initiative. The patient's main wishes were to consume certain foods that were not pureed and to consume beverages that were not honey-thickened. Hospital staff and family were able to provide him with some of those wishes, including food from Taco Bell, a doughnut, and a peanut butter sandwich. However, Julia and Taylor continued to ask what else would help the patient and family during this transition period, and Julia offered to take pictures of the family holding the hands of the patient and placing them in a small frame. The family was delighted at that idea but then declined as they were embarrassed at the condition of the patient's nails (long and untrimmed, stained and dirty). Undeterred, they offered to give him a manicure for the photo opportunity. They soaked, scrubbed, and filed his nails so that his family could have a wonderful momento before his passing. Julia also printed an EKG strip (name and identifying information removed) and placed it in a bottle for the family to have as an additional momento. Both Julia and Taylor went above and beyond the Schneck Standards (which are already set with high expectations) to provide this patient and his family with the most personal, comforting, and peaceful end. Even the family continuously thanked the team for their "above and beyond" measures for this family.
