Julie's extraordinary nursing skills are not limited to typical medical care; her unwavering dedication to communication, patient advocacy, and encouragement has a profound impact on patient outcomes.
As a Nurse Case Manager for PACT Team 15, Julie plays a vital role in addressing patient inquiries, concerns, and various needs that emerge between appointments. She also sees patients directly for procedures like vital checks, EKGs, and administering injections.

Recently, a veteran arrived at our clinic late on a Friday. He was in extremely poor health with multi-organ disease. He hadn't been seen recently at the VA and was new to our entire team. Hospitalization was recommended, however, he and his wife insisted upon going home. Julie was able, however, to at least have them agree to come back to our clinic on Monday. When he failed to appear, Julie contacted him and discovered he was reluctant to return due to fatigue. This was clearly caused by his medical condition. Julie persisted, encouraging them and communicating our concerns throughout the day. Eventually, she persuaded them to visit a community emergency room as he lived too far away to get to the VA before our urgent care center closed.

The next morning, Julie continued her efforts. He wasn't answering his home phone, but she eventually got a hold of him through family members. She found out that he had gone to a community emergency room as recommended. However, he left the emergency room prior to being seen because of the long wait. So once again Julie persisted with her encouragement, urging them to come back to see us at the VA. By this time, she had built up quite a rapport with the veteran, and with her urging, he finally agreed. His medical condition had worsened, but we were hopeful we had caught things in time, and now he agreed to be admitted to the hospital.

During the veteran's hospitalization, Julie and our primary care team visited him and collaborated with the inpatient team. After a week, the veteran was discharged, still very sick but significantly improved. Julie went above and beyond by making sure he understood his medications and discharge instructions, double-checking everything, and helping him understand all the details. Her diligent care and communication were critical in preventing a major medical crisis. After several months he had greatly improved and was doing well.

Julie took responsibility, and her persistent efforts made a life-saving difference.
