On Unit 56 we had a 93-year-old patient who was admitted for stroke. Prior to admission, this patient was functionally independent and able to care for herself. After her stroke, she lost her ability to move her left side and had difficulty swallowing. Unfortunately, she failed her swallow study and was unable to take anything by mouth without aspirating. Her daughter declined G-tube placement based on her wishes and she was transitioned to comfort care with comfort feeds. That evening at the change of shift, the patient's nurse, Julienne, noted a change in the patient's condition. She had been alert and talking throughout Julienne's shift but now seemed to be taking a turn for the worse. Julienne attempted to call the patient's daughter to make her aware of the patient's declining status but was unsuccessful in reaching her. Fearing the patient would pass away alone, Julienne attempted to call the NODA volunteer program but could not reach anyone. Julienne then decided she would stay with the patient after her shift was over. She reached out to the Chaplain who came to the unit and recited bible verses with the patient, at her request. Throughout the evening, the patient kept crying for her daughter so Julienne stayed to provide comfort. She stayed 4 hours past her shift to stay with the patient despite having to work the next day. She passed away that night. We weren't able to contact her daughter until the following morning and it gave her great comfort knowing that the staff had stayed with her mom.
