Mr. C was a patient for a number of months recently, having multiple surgeries and complications, including renal failure and multiple infections. He transferred from a regular floor to the SDU and to ICU and back and forth multiple times. Kara took care of Mr. C on and off during his stays on J1 SDU and became trusted by the patient and the patient's family. Kara knew the patient better than most, and as his stay was very complicated, she was an excellent resource for the rest of the staff and resident physicians.The son of Mr. C was his father's advocate and knew everything about his father's care. The son would inquire about current treatment and lab results and spent many, many nights at his father's bedside. It was obvious he loved his father very much. He was very intense, often asking endless questions about lab work, test results, potential treatments and all the risks involved, and yet Kara exhibited extreme patience and compassion with the son and the family. Many times the son was approached by physicians about the gravity of the patient's condition and a code status. The patient continued to deteriorate and the family wanted aggressive care.I witnessed Kara speaking with the son one night. She was completely honest with him and explained what would happen if in fact Mr. C's heart were to stop. Though not harsh, Kara's words were honest, clear and concise, and she phrased it such that the family could grasp the suffering that may occur. Mind you at this point the patient was not speaking or responding to his family. The family decided, with Kara's help, to make their father a DNR/DNI with active treatment, and Kara facilitated having the MD come and fill out the proper paperwork. That night the son went home for the first time in a long time and slept. I witnessed him thanking Kara for helping them make the most difficult decision, and express his gratitude that they could move forward towards acceptance.A few nights later, as the patient continued to deteriorate and the family had to decide whether to insert yet another line for pressors, they were blessed to have Kara as their nurse. Kara spoke with the son and the physician, and they made the decision for comfort care. Though Kara is a relatively new nurse, she showed such grace, confidence and compassion that I feel she fully represents what the DAISY award stands for. I am honored to work with Kara, and Morristown is blessed to have her.
