When we opened the floor I was very pleased that Kate Patton decided to be our case manager! Kate always goes above and beyond her job responsibilities and she does it with such passion. I have personally witnessed her fighting for our patients to get the necessary care after they leave the hospital. Recently she told me a story that she was speaking to the spouse of a patient who was being discharged to a SNF and the person sounded very ill. She told Kate that she was also positive for COVID-19 and was not feeling well at all but did not have any family to help her out. Kate offered to go to the grocery store for her to get some necessities. Kate told me that she also included some extras like soup and Gatorade. It meant a lot to the person and Kate did it out of the kindness of her heart.
