Kathleen has gone above and beyond to help a very special patient. This patient has multiple health problems stemming from his obesity. Complicating the situation, this patient and his mother have significant learning delays, emotional issues and live in poverty. Kathleen has worked with this patient in the school clinic on a daily basis, educating him on his medications and empowering him to take charge with his treatment. She has had multiple meetings with his mother, also to educate and empower. Kathleen has even bought him a gift card to Walmart to help motivate him in taking his medication. Kathleen has been instrumental in coordinating care with the specialists, his PCP, Children's Hospital, home health, HCP program and his school. She has treated this patient and his family with such respect and care. Because of Kathleen, this patient and his mother are learning to take care of the health problems that need to be addressed. This is just one example of how Kathleen treats her patients from the heart and is a facilitator in health.
