Kathy Carboni, RN, BSN, ACM

Kathy, while a team lead in her department, is one of the best Case Manager RNs I have ever worked with. Her attention to detail and understanding of post-hospital services ensure our patients discharge with strong plans to help them be successful. She seemingly single-handedly managed the Care Management department during a period of limited staffing with grace and patience, ensuring the patients still received all the care they needed and that the bedside staff didn't feel the vacancies in the Care Management team. Since she has devoted herself to finding the best outcomes for the patients and provides direct patient care, I want to nominate her for The DAISY Award.
