Because of Katie’s fierce advocacy, attentiveness, and incredible nursing judgment, this patient was able to not only receive this necessary admission, but also lifesaving antibiotics and definitive care.
Katie assumed care of a veteran who presented to the ED via EMS for hematuria and abdominal pain. After a somewhat benign workup, the MD, Resident MDs, and Pharmacist found that the patient was fluid overloaded, and they were planning on diurising the patient and discharging him back to his independent living facility. As soon as they voiced this plan, Katie firmly spoke up that this was not appropriate, as the patient was very ill appearing, had a new oxygen need, and would be discharging back to an independent living facility with nobody to check up on him should he continue to get worse. The veteran’s labs had been mostly unremarkable, and vitals were within normal limits. The patient had been afebrile on arrival. Approximately 3 hours after this patient’s arrival, while repeating an assessment, Katie found that her patient had spiked a fever. Going from 98.0 to 101.8. He had also become diaphoretic, tachypneic, and tachycardic at this point. After going back and forth with the providers, and these new findings 3 hours later, Katie had finally made a strong enough case for admission. Not only did the providers adjust their stance on the admission, but the Sepsis protocol was also initiated. Because of Katie’s fierce advocacy, attentiveness, and incredible nursing judgment, this patient was able to not only receive this necessary admission, but also lifesaving antibiotics and definitive care.
