I would like to nominate Kelly Harper from MS1 for the DAISY Award. She identified that pharmacy had listed a patient's home meds to include Apidra and Byetta, both antidiabetic medications. Upon researching these medications, she found they should never be prescribed together as they can cause renal failure. Kelly then involved the patient who clarified that Byetta was not a home med, but Biotin (an over the counter medication that promotes nail growth) was the correct medication patient was taking.

Kelly took the initiative to research and rectify this patient's medication reconciliation list. This action definitely promoted patient safety.
