Keri is the leader of the Complex Care Team. Her dedication to her team is immeasurable. She has taken on the task of closing care gaps for our patients in the Community Physician Network and this is no small task. She and her team were asked to change the course of their daily work to close thousands of care gaps by the end of the year. These closures help keep our patients safe and well in hopes to prevent more chronic disease states.

Keri leads this initiative alongside her team with a smile and positive attitude on a daily basis, rolls her sleeves up, and does it right along with her staff. This support she provides carries over to her team which allows them to strive to reach more and more patients and affect their health and well-being. Keri is a true asset to Community Physician Network and Community Health Network as a whole.
