Kristie was visiting Barker Cypress clinic and was helping with Med Reconciliation (already above and beyond). When reviewing a patient's medication, she noticed there were 2 different pills in the same bottle. One was his phos. binder sevelamer and the other was metoprolol. His doses were just increased. Both are white oblong pills but do very different things. Kristie separated the 2 pills, fully educated pt on how to store and take his meds. She also reminded him of what each did. She took extra time to review the patient's meds carefully and potentially saved him from an adverse event. He didn't realize they were 2 different pills and couldn't see the difference. He's never mixed bottles again.
