Krystal immediately notified the provider and the research nurse, ultimately the patient was withdrawn from the study.
The investigational study "IVIRONMAN " is new to us. This study is a double-blinded study where patients whose hemoglobin is less than 9 get to be in a study. They are expected to get a test dose and after one hour patients get the iron/placebo transfusion. Premedication is not required but if they have a history of asthma they will get a dose of steroids before the test dose. Patient C was a candidate for the study and since she has a history of asthma solumedrol was ordered as part of the protocol. But the patient has a documented allergy to BECLOMETHSONE. When Krystal came on shift she got the report that she needs to give the study meds to the patient. The patient was a late entry to care and from Honduras. She had to call the interpreter services to explain and assess the patient knowledge about this study she also had a chance to further investigate the type of allergic reaction the patient experienced with beclomethasone. According to the patient, “her throat started closing”. Krystal immediately notified the provider and the research nurse, ultimately the patient was withdrawn from the study. This near-miss incident didn’t happen because Krystal took her time to talk and investigate with a help of an interpreter. Krystal exemplifies nursing excellence by advocating for her patient which in turn prevented a potentially life-threatening incident.
