A patient on Renal Gold requested that his nurse be nominated. Our patient is 19 years old, homeless, with not a penny to his name. On discharge day, he had so many challenges with his home medications, specifically the antibiotics. Lara worked tirelessly to get them covered through his insurance. Pharmacy was denying it because the patient did not have a copy of his insurance card, including other pertinent paperwork. Lara created a care package for the patient. I could see on the patient’s face how grateful he was to be cared for by nurses like her. She has a big heart for those patients who need more resources.
