Latorian (Tory) Young joined the Home Assist team in January of 2023 as a per diem RN to work the HITH program with our partners at Metro. The HITH program is a relatively new program in which the patients are admitted to the hospital but are receiving care at home. The patient receives two nursing visits daily; all other medical support is virtual (MD and Virtual nurse). As the boots on the ground, Tory applies her extraordinary nursing skills to provide a thorough assessment and advocate for each patient she sees. She takes the time to ensure the patients have the tools to manage their illness in an effort to keep them out of the hospital and improve patient outcomes. She provides feedback to the team to improve the program. Over the past year, we have added a nephrology outpatient component and sickle cell crisis component; she has helped with program development, keeping the patient well-being the focus of all of these innovative programs while minimizing risk to the organizations and the patients we serve. I have learned a lot from Tory, and it has made me a stronger nurse and manager. It is a pleasure working alongside her.
