Because of Lisa, many patients have had the opportunity to be successful PD or HHD patients when they were struggling at In-Center.
Lisa Chelf BSN RN is a Regional Home Program Manager and GFA in the Lakeshore Division’s Region. Lisa has been a chronic dialysis nurse for over 27 years. With her nursing skills, understanding of dialysis processes, and commitment to patient care, she soon was given leadership and management responsibilities, but she has always maintained her skills in direct patient care. Lisa has long been an advocate for patient rights and patient self-determination, believing that all patients should have access to the dialysis modality of their own choosing. She has frequently engaged physicians to consider (or reconsider) making one of the Home Dialysis modalities available to patients that were being otherwise identified as a “poor candidate” for Home. For years, she has understood that traditional in-Center Hemodialysis is not the best option for all patients, especially for patients that do not function well in groups or in social situations. Because of Lisa, many patients have had the opportunity to be successful PD or HHD patients when they were struggling at In-Center. Additionally, Lisa has welcomed and advocated for patients in her home programs that had could no longer treat in-center due to psychosocial concerns. Lisa plays full out to ensure that her teams have the full resources that they need to provide great care. Lisa is known for her commitment to home programs near and far, traveling distances: supporting them, building them stronger, and creating networks for them. All of this she does with a two-fold goal: driving team development and providing the best possible care for patients. Although her primary responsibilities have been with home modalities for the past 10+ years, she continues to work very closely with our in-center teams, as well. She maintains strong clinical and leadership responsibilities with them. She is always present to help guide them through CMS survey processes and clinical improvements. Lisa’s efforts at creating a sanctuary for teammates and patients not only go beyond her assigned duties and employment but extends much further. Through the IPRO Network of the Ohio River Valley, Lisa extends her expertise and influence at a broad scale level as an elected member of the Medical Review Board. In this capacity, Lisa continues to drive the values that she shares with DAISY throughout our entire ESRD Network. Lisa is a true DAISY Nurse.
