I am proud to recognize Logan, RN, Care Manager, for his exceptional commitment to patient safety, clinical leadership, and compassionate care.
During a very normal day. Logan noticed subtle shifts in a patient’s behavior and emotional state that signaled a serious and immediate risk of self-harm. His ability to recognize these concerns, rooted in strong clinical assessment skills and deep attentiveness, set in motion a critical intervention.
Rather than treating the interaction as a standard encounter, he paused, listened closely, and created space for the patient to share what she was experiencing. Once he understood the urgency of the situation, Logan, along with another caregiver, acted without hesitation. He coordinated with the social worker, ensuring the patient was supported every step of the way.
Together, they accompanied the patient to the Emergency Department and remained a steady, reassuring presence throughout the entire process. He stayed with the patient through triage and admission, offering calm guidance, emotional grounding, and advocacy during a frightening and overwhelming moment.
He responded immediately and appropriately, ensuring the patient received urgent care and was never left without support. His ability to identify risk based on subtle behavioral cues reflects deep clinical knowledge and exceptional nursing judgment.
